Take a peek at grasshopper.com.  I use the cheapie $12/month option.  1 local number etc.
Unlimited extensions.  Easy to setup.  

If you want to support local, www.versature.com
At the time we didn’t go with them because they couldn’t support international call forwarding.
May have changed now.
